NP-NCD programme introduced structured reporting formats and registers to document screening, diagnosis, treatment, referrals, and follow-up services across healthcare facilities.
Purpose of Reporting
- Standardised Data: Comparable reports across all facilities and regions
- Reliable Planning: Inputs for drug supply, staffing, and policy making
- NPNCD Alignment: Recording and reporting at all levels must align with NPNCD guidelines
Reporting Flow
Recording and reporting formats for NCD programmes are prescribed by the Central NCD Cell and must be used uniformly across sub-centres, PHCs, CHCs, districts, states, and national level. Delays or errors at one level affect all higher levels.
