How does a patient move through the NCD clinic?

Registration in OPD
Age ≥30 registered. Unique ID and screening card assigned. Sent for screening.
Measurements and Counselling
Weight, height, waist circumference, BP, and random blood sugar measured. NCD risk factor counselling provided.
Diagnosis and Treatment
MO takes history, diagnoses hypertension/diabetes, initiates treatment. Doctor advises on follow-up schedule or referral if needed and medication adherence.
Follow-up and Referral
Patient receives medication and visits again as per the schedule.
Health Care Staff Responsibilities
Effective NCD control requires every cadre of health workers to play their defined role in the care flow. A missing link at any level breaks the chain of care. Following healthcare staff members are involved at PHC:
